Parental Consent Form

Employers' Request for Child Labor
(RSA 276-A:5 and LAB 1001.04) This document requires the free Adobe Acrobat Reader available by clicking on this icon.

In order to obtain the NH Youth Employment Certificate, the Employer's Request For Child Labor form must be completed by the employer and brought to the school by the student. The school will then issue the NH Youth Employment Certificate to the student who will bring the Certificate to the employer to be kept on file.

An employer who has received a civil penalty or has been ordered to pay wages as a result of one or more violations on an inspection report may request a hearing within 30 days of receipt of the letter from the Wage and Hour Administrator. A guide has been developed in order to assist the employer as a quick reference to the hearing process.
