Workers' Compensation insurance is mandatory in New Hampshire for businesses that have any employees. This legislation protects employers as well as employees. This is why insurance companies are required by law to notify this Department of their intent to terminate coverage either thirty or forty-five days in advance, depending on the reason for termination. You may have already paid your insurance premium or may have made arrangements with your agent to change carriers. In that case your insurance company has an obligation to file the proper paperwork with this Department to assure that it does not appear that there is a lapse in coverage. We ask your assistance in completing this online questionnaire so that we can maintain the proper coverage records and follow up with your carrier if they have failed to file the proper coverage forms. This questionnaire must also be completed if you have gone out of business or sold your business. If you no longer have employees or are leasing your employees, this information must also be provided. If, however, your workers' compensation insurance is being terminated and you are engaged in business in New Hampshire with employees, you will be in violation of the workers' compensation law effective the date of the termination of your coverage.
